Map data is presented at the level of the planning neighborhood. While planning neighborhoods are larger geographic areas than census tracts, census tracts do not always lie completely within a planning neighborhood. SFDPH used ArcGIS software and a 'centroids within' methodology to convert census blocks to geographic mean center points. We then assigned census blocks to planning neighborhoods based on the spatial location of those geographic mean center points and calculated the planning neighborhood totals for the table.

Explanation and Limitations

Population density is one proximal but not comprehensive indicator for health care demand. See HDMT Demographic indicator D.1 for neighborhod population density calculations: http://www.thehdmt.org/indicators/view/183

By default, in communities with more people, there will be more demand for health care services. However, the type of services needed and the associated length and depth of care, is determined by numerous factors including the demographics of the community residents and their access to preventative health services.

Demographic shifts brought on by changing housing and economic contexts will also result in patient demographic shifts in clinics and hospitals and the associated health care utilization. Older populations tend to have longer, in patient care than younger populations. Populations without access to health insurance tend to arrive at health facilities in a more advanced stage of disease or illness progression, thus necessitating inpatient visits, than persons with health insurance. Younger populations tend to utilize obstetric and emergency room services more frequently, whereas older populations tend to utilize internal medicine, surgical and geriatric services more frequently. Thus, in addition to examining population density, it may be helpful to analyze the age, gender, racial/ethnic, and socio-economic profiles of neighborhoods relative to the neighboring health facilities.

Additionally, quantitative and qualitative studies could be conducted to analyze where people go for their health care. Physical proximity to health facilities is just one aspect of access to health care. Other factors such as health insurance coverage, transportation to and from the health facility, cultural competency or cultural humility of health care providers, hours of operation, length of reimbursement period, cultural and linguistic competency of administrative and intake staff, availability of child care, availability of prevention programs, and employer requirements are among many factors impeding reliable, continuous access to affordable, quality health care.

This list of facilities includes all known public health facilities who provide primary care in San Francisco. This list was created by combining health care clinics and hospitals included in San Francisco's Community Health Network (CHN) and the list of facilities licensed by the California Department of Health Services, Office of Statewide Health Planning and Development (OSHPD). Consolidated hospital licenses (i.e., CA Pacific Medical Center) are listed as separate locations (i.e., East Campus, Davies Campus, Pacific Campus and West Campus) when there are distinct facilities.

The types of facilities include:

Clinics included on the OSHPD list include facilities that are licensed under Section 1204 of the Health and Safety Code (indicating that they are a 501c3 non profit organization and operate as a primary care clinic). These clinics are ones that operate for more than 20 hours per week and are considered to be an independent facility. Under this criteria, small school clinics and clinics within a larger hospital setting are excluded. Additionally OSHPD criteria excludes tribal facilities located on tribal land (not applicable in San Francisco), county facilities and non 501c3 (or for-profit) health facilities. Although county facilities are excluded on the OSHPD lists, San Francisco County public health facilities were included on our list of facilities providing primary health care.

Facilities included on the list may be on both the OSHPD and CHN lists and may offer a variety of services. For example, the two public hospitals - Laguna Hospital and San Francisco General Hospital offer various clinics geared towards particular populations (i.e., for women, children, rehabilitation, dialysis, etc). However, these facilities are listed only once, according to the primary purpose of the health facility (generally primary/acute care).        

The following facilities were not included on the list: home health agencies, hospices, private doctors'offices, specialty clinics, intermediate care facilities for the developmentally disabled, referral agencies, skilled nursing facilities, chemical dependency recovery hospitals, alternative birthing centers, psychiatric health facilities, and certified but not licensed facilities. Additionally the American College of Traditional Chinese Medicine and other traditional Chinese and alternative medicine providers are not included. Some of the listed primary care facilities and clinics may offer services provided by specialty clinics on the facility grounds.  

Using this exclusion criteria, the following clinics are not included in the list above: the SFGH women's health center, SFGH general medical clinic,SFGH children's health center, SFGH Family Health Center, SFGH Ernest Gallo Clinic and Research Center, St. Luke's Breast Clinic, St. Luke's Orthopedic Clinic, St. Luke's Women's Center, St. Luke's Pediatric Clinic, the Transgender Clinic (at Tom Waddell Clinic), Adult Immunization and Travel Clinic at SFDPH/101 Grove Street and the STD Clinic at 356 7th Street.  The Hawkins Village Teen Health Center and Hip Hop to Health Clinic which serve teenagers were also not included because they are open less than 20 hours per week.

Changes in population density will likely have an impact on demands for health care at local facilities.
